MUTUAL CONSENT AGREEMENT

Agreement

By signing this document, both parents/legal guardians agree to the following terms:

1. Post-Admission Authority: After the child has been admitted to Foundation World School,

Illahibagh, either parent (mother or father) may act as the child’s guardian in the absence of the

other. This includes but is not limited to attending parent-teacher meetings, making sure all

financial obligations are met, participating in school events, and making routine educational

decisions.

2. Emergency Decisions: In case of emergencies or urgent matters requiring immediate attention,

either parent has the authority to make decisions in the best interest of the child, even in the

absence of mutual consent.

3. Continued Collaboration: Both parents agree to continue collaborating on major educational

decisions throughout the child’s time at Foundation World School, Illahibagh. They commit to

resolving any disagreements through dialogue and, if necessary, mediation.

4. Notification of Changes: Either of the parents agree to promptly notify Foundation World School,

Illahibagh of any changes in their relationship or living arrangements that may affect their ability

to make joint decisions. This includes changes such as separation, divorce, or relocation. In the

event of a change in your relationship, you are required to inform the school immediately.

Additionally, the parents must submit a formal application designating which parent will act as the

legal guardian for the child. This application should clearly outline who will be responsible for

making decisions related to the child's education and well-being. If you are unable to reach a

mutual agreement on the legal guardianship, please be aware that your child’s admission status

will be placed on standby. The admission will remain on hold until a definitive decision is made

and communicated to the school.

5. Custody Documentation Requirement: In situations where the parents of a student are separated

and one parent has been granted custody, it is mandatorily required that the custodial parent

submit the relevant legal documents (negligence of which shall not be the responsibility of the

school). These documents should clearly indicate which parent—mother or father—has been

awarded custody of the student. This information is essential for the school’s records and to ensure

the appropriate parent is contacted regarding the student’s well-being and educational matters.

6. Nomination of a Legal Guardian: It is mandatory for both parents to mutually decide and

nominate a legal guardian for their child. The nominated person’s Aadhaar card, along with this

agreement duly attested, must be submitted to the school authorities for validation and recordkeeping.

The appointed legal guardian shall act in the capacity of the parents and be granted the

authority to exercise all the aforementioned responsibilities in their absence.

Acknowledgment

By signing below, both parents acknowledge that they have read, understood, and agreed to the terms

outlined in this document. They commit to upholding the principle of mutual consent for the child's

admission and agree to the provisions for one parent acting on behalf of the other post-admission.
